When gathering documents and receipts
for filing
 income tax returns, watch for two key
AMPI forms.
Each member receives an annual statement
detailing 12 months of
milk check receipts.
The statement also includes pounds of milk marketed, gross
earnings and deductions.
Form 1099-PATR is also mailed to each member, outlining taxable
allocations received from AMPI in 2007.
Members who don’t receive an annual statement or the 1099-PATR
within a week after the Jan. 31, 2008, deadline, should call
